All student-athletes must complete the required forms and have a current physical on file before participating in any sports activities.

The SCUSD physical form must be filled out by a doctor. If you do it through your regular insurance provider, please be sure to schedule early. The clearance is good for one year after completion.

Paper Packets

If you are unable to complete the athletic clearance requirements online, please contact Athletic Directors Brittney Codera (brcodera@scusd.net) or Gil Vasquez (givasquez@scusd.net) to arrange a paper packet.

STEPS HOW TO GET CLEARED FOR ATHLETIC PARTICIPATION

Step 1 – Go to www.athleticclearance.com

Step 2 – Select “California”

Step 3 – If you have an account, login – if you do not have an account, register for one

Step 4 – Click the “Start Clearance” button

Step 5 – Select “Kathleen Macdonald” or “MacDonald” and select your sport(s)

Step 6 – Fill out the information asked for, read and sign ALL required documents (no student ID from MacDonald, put 123456 for now, but you have to go back and change it)

Step 7 – Upload your completed physical AND insurance card photos. Link them to the downloadable Physical Form found HERE.

Step 8 – Push Submit

**If you are still in need of a physical for the current school year, please understand that you CANNOT participate in any active sport (practices, tryouts, games) without a physical on file**

**You must have an email with a green CLEAR next to your name in order to be considered athletically cleared**

**Please call your physician or any local walk-in clinic to complete your physical ASAP**

**Did you get a message of PRACTICE ONLY or DENIED on Athletic Clearance? Read the email that tells you what you need to fix and complete that. Then email Mrs. Codera (brcodera@scusd.net) or Mr. Vasquez (givasquez@scusd.net) to confirm the completion**

 

Eligibility

To be eligible to play a sport at MacDonald High School an athlete must:

  1. Be physically cleared to play
  2. Be passing a minimum of four classes
  3. Maintain good behavioral standing as outlined in our student handbook
  4. For the previous grading period, the student athletes must have been enrolled in 20 units and received a 2.0 GPA. If they received a 1.5-1.99 GPA they may apply for  academic probation. If you are in this situation, please discuss the matter with your coach.
  5. Student athletes must not have turned 19 prior to June 15th of the preceding year.
  6. If a student athlete ever attended a different high school, it must be determined whether or not CCS transfer paperwork should be filled out. Please speak to your coach or an athletic director to determine what steps need to be taken.

This is not a comprehensive list of requirements, but rather a few other reminders of some of the requirements to be eligible to play a sport.

  • Academic Eligibility: Student-athletes must have earned a minimum 2.0 GPA. If their GPA was between 1.6 and 1.99, they may apply for academic probation. If this applies to you, please talk to your coach.
  • Transfers: If a student-athlete has attended a different high school, CCS transfer paperwork may be required. Please check with your coach or the athletic director to determine next steps.
